This role involves carrying out home visits, assessing the needs of older people, including those with mental health needs , people with physical disabilities, and their carers needs in accordance with the Care Act 2014
You must have a genuine interest in promoting independence and supporting people to regain their confidence and skills.
You will be required to develop and monitor reablement programmers to empower people with activities that are important and meaningful to them.
You will work within a multi-disciplinary team including care agency staff, occupational therapists, and physiotherapists and social workers to help people attain an agreed level of independence.
An understanding of reablement / rehabilitation and adult social care is desirable
