The Change Lead for the Care at Home Transformation programme will have the opportunity to work with us on fully transforming Care at Home with the introduction of a population based commissioning model, flexible care and support, built around people’s strengths and the outcomes they wish to achieve. We want to put people in the driving seat and need your energy and skills to help deliver change that will make a difference to people lives.
You will need vision, energy and a strong belief that people know best what works for them, and you are committed to delivering the change that will make it happen. You will need to be organised, structured, a good communicator, an influencer and above all someone who enjoys delivering change, that works.

Candidate Profile
Candidates will be required to have extensive knowledge of health and social care including the ability to apply change management expertise to help deliver The Care at Home Transformation related to improving personalisation in Adult Social Care. Candidates will support the Head of Integrated Commissioning for Older People to work collaboratively with local people, staff and partners to implement change through redesigning our approach to delivering personalisation.
Candidates will be confident in their practice and values and be able to inspire and motivate staff and Partners to engage in a shared form of service delivery. Candidates will be expected to have extensive knowledge around social care policy and legislation, particularly the Care Act 2014 and Mental Capacity Act 2005, strengths-based practice and personalisation. Candidates will be expected to be confident in working and engaging collaboratively with local people and providing advice, guidance and training to staff and Partners. Candidates will be able to tailor their approach to different Partners, understanding their strengths and supporting them with areas identified for improvement.
