Brokerage is a critical function in supporting residents to meet their assessed care and support needs. As a Care Broker, you will work closely with adults, their families, and practitioners to identify appropriate services and self-directed support options that promote independence, choice, and wellbeing in line with the Care Act 2014.

    • Develop and maintain strong knowledge of local services and community resources.
    • Work collaboratively with colleagues across Adult Social Care and partner agencies to support timely and effective care arrangements.
    • Complete brokerage reviews for new care packages to ensure they are meeting identified needs and outcomes.
    • Ensure accurate and timely recording of brokerage activity in line with Council processes and systems.

Please note that the successful candidate will be required to be office based at least 4 days a week

    • Knowledge of self-directed support, personal budgets, and person-centred care planning.
    • An understanding of safeguarding responsibilities and the Mental Capacity Act.
    • The ability to communicate clearly and sensitively with individuals, families, and professionals.
