Care Broker - Adult Social Care
The London Borough of Bexley is seeking a motivated and committed Care Broker to join our Community Commissioning Team within Adult Social Care.
Brokerage is a critical function in supporting residents to meet their assessed care and support needs. As a Care Broker, you will work closely with adults, their families, and practitioners to identify appropriate services and self-directed support options that promote independence, choice, and wellbeing in line with the Care Act 2014.
You will play a key role in sourcing and coordinating care and support packages, ensuring that individual wishes and outcomes are central to all arrangements, while working within Council policies and ensuring best value.

Key Responsibilities
- Identify and arrange appropriate commissioned and non-commissioned services to meet assessed eligible needs.
- Support individuals to understand and exercise choice, including self-directed support options such as Direct Payments and Individual Service Funds.
- Provide advice and guidance to individuals, families, carers, and attorneys, including information on charging and financial contributions.
- Develop and maintain strong knowledge of local services and community resources.
- Work collaboratively with colleagues across Adult Social Care and partner agencies to support timely and effective care arrangements.
- Complete brokerage reviews for new care packages to ensure they are meeting identified needs and outcomes.
- Ensure accurate and timely recording of brokerage activity in line with Council processes and systems.

Candidate Profile
We are looking for someone who is person-centred, organised, and confident in working in a fast-paced adult social care environment. You will have:
- Experience of working with adults with a range of care and support needs, including older people, people with disabilities, and those with mental health needs.
- Knowledge of self-directed support, personal budgets, and person-centred care planning.
- An understanding of safeguarding responsibilities and the Mental Capacity Act.
- The ability to communicate clearly and sensitively with individuals, families, and professionals.
- Strong organisational skills and the ability to manage competing priorities.
- A commitment to promoting independence, choice, and wellbeing for Bexley residents.
Relevant health or social care qualifications are desirable, or you will have substantial equivalent experience and a willingness to undertake relevant training.
