Several permanent full-time and part-time opportunities are available within our Bexley Complex Care Team.
The Complex Care Team provides a multi-professional service for referrals received by Bexley ASC using a person centred and strength based model of intervention. Working closely with Health colleagues and our other services across Adult Social Care and Health.
The Complex Care Team within out localities provides ongoing assessment and case management to vulnerable adults within their homes, supported living and residential care. Often working with individuals to minimise their risks and support then to live more independently.
We require Social Care Assistants who can work effectively within a multi-disciplinary team and autonomously when required. Key requirements of the role include the ability to work at a fast pace and have experience and resilience to cope with demands of a busy team. We are looking for candidates who have a positive, forward-thinking attitude that can work collaboratively as part of a team. assessing the needs or residents (and that of their carer), to establish creative, yet practical care plans that promote choice, control, and independence. A background in the assessment and the provision of social care is highly valued.
