  • About The Role

  • Here at London Borough of Bexley, we are transforming our practice model to support our commitment to providing the highest level of care. Our determination to consistently achieve positive outcomes drives the changes we have made. We are now looking for professionals who share our passion to join us as we move towards an exciting future. We strive to promote independence, giving as much control to service users as we can, allowing them to make decisions that have such an impact on them. We firmly believe that all adults deserve to live their lives with as much dignity as possible. With increased investment and dedicated staff, we’re working to achieve exactly that.

As part of a growing team, you’ll help provide vulnerable adults in our community with brighter, more positive futures. This includes older people and those with physical and learning disabilities and mental health.
The Bexley Care Integrated Triage Hub provides a multi-professional screening service for all referrals received by Residents of the London Borough of Bexley using a ‘see and sort’ solution-focused model of intervention.  It provides a Duty response for the Complex Care Team.  Working closely with Health colleagues and the Rapid Response Team, the Triage Hub will deal with crises and emergencies as they arise.

We are looking for a qualified Social Worker with a firm grasp of Adult Safeguarding and of working in a multi-disciplinary environment.  You should have a clear understanding of the Care Act and of the national agenda on the integration of Health and Social Care.  You should be able to demonstrate a firm commitment to person-centred working and have a ‘can do’ approach to day-to-day challenges.

You’ll work as part of a multi-disciplinary team, assessing the service user’s needs (and that of their carer), to come up with a creative, yet practical, programme that will enhance their quality of life.

We’re looking for a qualified Social Work England registered Social Worker with post qualifying experience, having completed your AYSE award (if applicable). However, this role may also be suitable for a talented and dedicated Newly Qualified Social Worker. The most important things are that you genuinely enjoy working with vulnerable adults and can demonstrate your capabilities against the Social Work Professional Capabilities Framework (PCF).

You’ll be flexible, approachable and adaptable – towards service users and your colleagues alike – as well as being someone whose expert professional and legal knowledge is backed by a human touch. You will also be able to demonstrate experience in conducting Safeguarding Vulnerable Adults investigations. You may also be a qualified Best Interests Assessor or Practice Educator or be prepared to complete training in at least one these areas.

For more information and an informal chat please contact Wendy Douglas, Service Manager Triage Hub on 0203 045 3242.
